Dates to Remember:  

  • After May 17 there will be a mandatory/no exception $15 late fee per registrant.  Supplies for crafts and projects are not guaranteed.  Anyone registering after May 17 will also need to provide a 1:1 ratio - one adult to one scout.  Contact Denise Rubey @ seddaycamp@gmail.com for paperwork turn in options.

Required forms:  

 

Scouts, Siblings and Junior Staff:

1. Medical Form  MUST USE THIS 2014 FORM, OLDER FORM NOT ACCEPTED    *  Immunization Cheat Sheet - Enter month and year of latest shots, do not send in shot records.  Only list the LAST date immunizations were given.

2. Copy of insurance card

3. Transportation Pick Up Form

4. Service Agreement (JR Staff Only)

5. Letter of Recommendation  (JR Staff Only)


Adult Staff:

1. Medical Form  MUST USE THIS 2014 FORM, OLDER FORM NOT ACCEPTED

2. Copy of insurance card

3. Signed ‘Adult In Camp State Compliance’ form

4. Copy of Sex Offender Database Search results

5. Copy of First Aid or CPR certificate (if you have completed this training)

6. Copy of Range Training Certification (Range directors only)

7. Copy of Licenses or Certifications (Nurse station)

8. Copy of YPT (Youth Protection Training) card dated after 6/1/12.        Must be from an in-class training session.  Texas no longer recognizes the online training for camps.  There will be opportunities for training prior to camp.

 

 

All forms must be printed, completed, signed and submitted to your Pack Representative by agreed upon date.  All paperwork must be received by 5/31/14.  Any registration not paid by 6/1/14 may be dropped from the camp roster.  Online registrations are not complete until all required paperwork and payments have been received by Soaring Eagle District Day Camp Registration Director.

Important Pack Representative Registration information:

  • Turn in one check for your pack.  Make checks payable to SHAC and ref: account # 1 6801-687-20 Soaring Eagle Day Camp  on the check.
  • Attach check to Cover Sheet
  • Registration turn-in dates: April 1 6:30-8:30, April 10 6:00-8:00 and May 8 6:00-8:00
  • Turn in outside of the three dates listed above must be arranged with Denise Rubey
